CLIENT AND THERAPIST RELATIONSHIP
I will develop my relationship with you or your adolescent beginning with a 2-3
session intake/evaluation process. During this time we can both decide if I am
the best person to provide the services you need in order to meet your treatment
goals. In the first session I will discuss the limits of confidentiality, as
well as complete any paperwork, and sign any authorizations. If we agree to continue
and we begin psychotherapy, we will usually schedule our 50-minute sessions at
a mutually agreeable time. Sometimes sessions are longer or more frequent depending
on our schedules, your commitment, and the severity of the presenting issue.
I will work with you to develop the best approach to help you understand, confront,
and change those obstacles to your health and enjoyment that brought you to therapy.

purpose and serenity.
RECORDS
There is no charge for copies, faxes, or the mailing of records 5 pages or
less. For 6 or more pages there is a charge of 25 cents per page plus postage.
A completed, signed release of information is required.
PAYMENT
Payment is due at the time of service. I am unable to accept credit cards at
this time. I will accept cash or check. There is a $15.00 charge for returned
checks. Counseling sessions are scheduled by appointment only. You are responsible
for a fee of $50.00 for any appointment that you do not cancel 24hours in
advance.

My hourly fee is $100.00. I will provide sliding fee services on a case by
case basis. Please ask.

My practice is HIPPA compliant
HOURS
I provide daytime office hours Monday through Thursday and evening hours on
Monday and Tuesday.
EMERGENCIES
If you have an immediate, life threatening emergency, call 911 or promptly
go to your closest emergency room.
During office hours, I will return your call as soon as possible between individual
or group appointments.
NON-EMERGENT ISSUES
For non-emergent issues, I will return your call no later than 24 hours. If
I am unavailable please call Crisis Response
Services at 1-888-568-1112.
